Our duties

Berkins Home Care Services, LLC is required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and notify affected individuals of any breach of unsecured PHI.

How we may use your information

We may use and disclose your PHI to provide treatment, obtain payment for services, conduct healthcare operations, and as otherwise permitted or required by law.

Your rights

Right to inspect and obtain a copy of your PHI.
Right to request a restriction on certain uses or disclosures.
Right to request confidential communications.
Right to request an amendment of your PHI.
Right to receive an accounting of disclosures.
Right to a paper copy of this notice.

Filing a complaint

If you believe your privacy rights have been violated, you may file a complaint with us by contacting our Privacy Officer at info@berkinshomecareservices.com or by writing to 1101 Orchard Heights Dr, Mayfield Heights, OH 44124. You may also file a complaint with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
